Yorkville Transition Program

Our mission is to empower young adults with disabilities by providing a comprehensive, individualized transition program that fosters independence, enhances life skills, and prepares them for successful employment and active participation in the community. Through personalized support, vocational training, social skills development, and self-advocacy, we aim to help each student build confidence, achieve their personal goals, and thrive in the next phase of their life beyond high school. Our guiding principles will help students create a future where young adults with disabilities are fully prepared, confident, and empowered to lead independent and fulfilling lives.
